Santé Care Center assists you with completing an assessment of your patient’s health care needs once a year. This visit will be with a licensed nurse practitioner and they will:
Review and document all medical issues by completing a physical exam and asking about the patient’s medical history
Take a full record of all your patient’s medications
Complete a simple screening for peripheral artery disease (hardening of the arteries)
Complete an eye exam, if your patient is diabetic
Send you a complete report to assist with care planning
